Severity framework
A severity framework converts review evidence into an explicit launch disposition using user impact, likelihood, detectability, reversibility, ownership, and closure criteria.
A duplicate-charge path and a missing dashboard enter the same review
Ava and Ben sort two problems: a shopper might pay twice, and one chart is missing. They must stop launch for the dangerous one without calling every task an emergency.
